DIRECTOR OF NURSING- Full-Time (APPLY NOW)

We are currently seeking a dedicated, compassionate, and driven Director of Nursing & Clinical Operations to join our Clinician-owned and operated Hospice Company.

The Director of Nursing & Clinical Operations (DNCO) is a registered professional nurse who ensures the quality of patient care in the home. The primary function of the DNCO is for the overall administration of the clinical departments and monitoring of appropriate staffing and productivity. In addition to administrative oversight of the clinical staff (including RN Case Managers, Nurse Aides, Social Workers, Chaplain/Bereavement Coordinators) and Volunteers, the DNCO may additionally maintain a regular caseload as dictated by the needs of the agency. The DNCO will professionally and effectively coordinate care with the interdisciplinary group, patient, family, and referring agencies. The DNCO will also monitor and analyze QAPI issues and direct clinical staff regarding agency policies and procedures.To be considered for this position you will need to provide proof of the following credentials and documentation:

  • 3 years of hospice case management experience and 1+ years of management experience in hospice
  • TX RN license that is current and unencumbered
  • Reliable transportation with current insurance coverage
  • Current CPR
  • Current TB test with a negative result or the equivalent
  • Available to see patients and conduct regular supervisory visits within our agency's service area (Arlington, Fort Worth, Mid-Cities, Mansfield, South Dallas, and Ennis) 
  • Available to work 1 weekend of On-Call per month

*EMR experience, Consolo experience a plus.

RN CASE MANAGER - Full-Time (APPLY NOW)

To be considered for this position you will need to provide proof of the following credentials and documentation:

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
  • One year of experience in hospice/palliative care, public health, oncology, acute care, or home health nursing
  • Current and valid Texas RN License
  • Valid Texas state driver’s license (auto liability insurance required) with a qualifying driving record.

CNA (Certified Nursing Assistant) - Part-Time/Full-Time (APPLY NOW)

As a CNA with Hospice Care Partners, you will provide outstanding patient care and physical support for patients as assigned. To be considered for this role you should have the following credentials and documentation:

  • Texas nursing assistant certification
  • Current CPR certification
  • Current TB test (may be provided if needed)
  • Valid (non-expired) TX driver’s license
  • Proof of auto insurance
