Hospice Care Partners provides outstanding hospice care aimed at improving quality of life, dignity, as well as pain and symptom management. Hospice -- a word that is subject to misconception and to some has a negative connotation, but in reality is a very positive approach to coping with life-limiting illness that focuses on patient and family needs. Hospice care neither hastens nor delays the end of life.
Hospice, by focusing on the patient and their family's needs instead of the disease process, is often times able to help improve the quality of life for all those involved. Every plan of care is customized to each individual patient and is regularly under review by our Medical Director and Interdisciplinary Team (IDT).
We provide care in the privacy of your home; our medical staff come to you.
- Prescription drugs associated with your diagnosis can be ordered and delivered to your home for your convenience.
- Durable Medical Equipment (DME) may be brought to your home if necessary to help make the patient as comfortable as possible.
- Advanced directives are created and followed so that once the time comes, we can honor the patient's wishes thereby alleviating some of the stress during that difficult time.
We're here to support the families of our patient as well. Our Medical Social Workers are here to help take care of any issues that may fall in their scope of expertise. Our Chaplains on staff are available to assist patient and family with spiritual needs and to follow up with our 13 month bereavement program.
All is Well
Death is nothing at all. It does not count. I have only slipped away into the next room. Nothing has happened. Everything remains exactly as it was. I am I, and you are you, and the old life that we lived so fondly together is untouched, unchanged. Whatever we were to each other, that we are still. Call me by the old familiar name. Speak of me in the easy way which you always used. Put no difference into your tone. Wear no forced air of solemnity or sorrow. Laugh as we always laughed at the little jokes that we enjoyed together. Play, smile, think of me, pray for me. Let my name be ever the household word that it always was. Let it be spoken without an effort, without the ghost of a shadow upon it. Life means all that it ever meant. It is the same as it ever was. There is absolute and unbroken continuity. What is this death but a negligible accident? Why should I be out of mind because I am out of sight? I am but waiting for you, for an interval, somewhere very near, just round the corner. All is well.
