- Our new home page includes a new "chat live" feature where you can start a conversation with a member of our team. Just respond to the "How Can I Help?" prompt in the bottom right portion of your screen.
Request a Consultation about Hospice:
- If you are a patient, caregiver, or family member/friend with questions about hospice, you can now request a no-obligation consultation with our team to learn more about how hospice care help.
- You can request a consultation and even book your preferred time to speak with us seamlessly online.
Register for an Upcoming Event/Follow Our Blogs:
- As an agency, we believe that education (and FREE access to it) is key. You can now register for upcoming informational webinars and events directly through our website.
- We have also launched a series of community and hospice specific blogs to provide you with additional information about hospice and to connect you with others in the community with similar questions or needs. Hospice helps everyone.
Submit a Secure Referral Online (Physicians & Facilities ONLY):
- Physicians & Facilities can now submit secure online referrals to Hospice Care Partners. By selecting the "Physician's & Facilities" tab from our main webpage, you can submit a referral including uploading patient documents and information.
- We understand the time-sensitivity of getting your patients on service. That is why we have built out this new referral submission platform to make it easier for you to refer to us and to expedite our speed to care.
